A Clinical Study of Henoch-Schonlein Purpura after ¥â-hemolytic Streptococcal Infection

Abstract
A clinical study had been performed on 134 children with Henoch-Schonlein purpura,
experienced at Severance Hospital during 6 years 5 months from January 1981 to May
1987. Based on clinical manifestations, 62 children had been diagnosed as
Henock-Schonlein purpura due to beta-hemolytic streptococcal infection(infection group),
and 72 children had been diagnosed as Henoch-Schonlein purpura due to other
etiologies(control group).
The results of this study were summarized as follow :
1) The sex ratio(M : F) of streptococcal infection group was 1.95 : 1 and control
group was 1.06 : 1. Males were predominant in streptococcal infection group than in
control group.
2) There was no difference in age, seasonal incidence, hematologic findings, previous
history of URI and allergic history between streptococcal infection group and control
group.
3) Purpuric skin manifestation was more prevalent in streptococcal infection group
than in control(p<0.01). On the other hand, petechia and ecchymosis were more
prevalent in control group(p<0.05).
4) Abdominal manifestations especially melena(p<0.05) were more prevalent in
streptococcal infection group than in control group(p<0.01).
5) Joint manifestations and kidney manifestations were more prevalent in streptococcal
infection group than in control group(p(0.01).
6) Recovery period of skin and abdominal manifestations was longer in streptococcal
infection group than in control group(p<0.01).
7) Relapse rate in Henoch-Schonlein purpura was higher in streptococcal infection
group than in control group(p<0.01).
This study suggests that clinical manifestions of Henoch-Schonlein purpura after
be-ta-streptococcal infection were more severe than control group and relapse rate was
higher in streptococcal infection group than in control group.
